Privacy Choices and Requests

This page explains how to exercise privacy choices regarding personal information controlled by Meridian Vitality LLC. The rights available to you depend on applicable law and the type of information involved.

Available requests

You may ask Meridian to:

• confirm whether we process personal information or consumer health data about you;
• provide access to or a copy of information;
• provide information about categories, sources, purposes, and recipients;
• correct inaccurate information;
• delete information;
• withdraw consent for future collection or disclosure where processing is based on consent;
• opt out of a sale, sharing for cross-context behavioral advertising, or targeted advertising, if applicable; or
• reconsider a decision by submitting an appeal.

Meridian does not sell consumer health data, does not use or disclose consumer health data for targeted advertising, and does not sell personal information for money.

Submit a request

Use one of these methods:

In your request, provide your name, the email address or telephone number associated with your account, the right you want to exercise, the type of information involved, and enough detail for us to understand the request. Do not send medical records, full payment-card numbers, Social Security numbers, passwords, or identity documents through ordinary email. We will provide a secure method if additional sensitive information is needed.

Verification and authorized agents

We may verify your identity by matching information you provide with information already maintained by Meridian, asking you to authenticate through an existing account, or using another commercially reasonable method. Verification information will be used only to process and document the request.

An authorized agent may submit a request where law permits. We may require proof that the agent is authorized and may contact you directly to verify identity and authority. A parent, guardian, conservator, or holder of a valid power of attorney may be asked for documentation of that status.

Timing, fees, and appeals

We will respond within the period required by applicable law. Many privacy laws allow up to 45 days, with one additional 45-day extension when reasonably necessary and properly disclosed. Requests generally are free. We may charge a reasonable fee or decline a request only where applicable law allows, such as for a manifestly unfounded, excessive, or repetitive request.

If Meridian denies a request, the response will explain the basis where required. To appeal, email support@meridian-vitality.com with the subject line "Privacy Appeal" or "Consumer Health Data Appeal" within 30 days after the decision and explain why it should be reconsidered.

Marketing and cookie choices

• Use the unsubscribe link in a marketing email to stop marketing email.
• Reply STOP to stop marketing text messages.
• Adjust your browser settings to block or delete cookies. Some Services may not work correctly if necessary cookies are blocked.
• Where required by law, Meridian honors recognized browser-based opt-out preference signals, such as Global Privacy Control, for the browser or device sending the signal.

Opting out of marketing does not stop non-marketing communications about accounts, requested services, transactions, appointments, safety, or legal notices.

Clinical records and third-party information

A healthcare provider, physician group, pharmacy, laboratory, payment processor, or other independent organization may control information under its own privacy notice. Meridian may forward or help route a request, but the organization controlling the information may need to respond directly. For clinical records or HIPAA rights, contact the treating provider using the information in the provider's Notice of Privacy Practices.
